Instructions for filling out a Request for Extension of Postdoctoral Appointment Beyond the 5th Year

Appointment Extension Request
The Postdoctoral Fellow and Faculty Advisor should work together to describe the need for additional training and the outline of the training plan that will be followed during their appointment extension. Examples are included below, please note that these examples are not exclusive and that other training plans may also be acceptable.
Here are some examples of training plans:

Dr. Postdoc has just received a K99 career transition award from the National Institutes of Health and will require an additional year of postdoctoral training to carry out the planned research in preparation for moving on to the independent, R00 stage of this award. Specifically, Dr. Postdoc will be establishing [some new line of research] under the mentorship of [PI, other mentors] that they can use as the foundation of their independent work.

Dr. Postdoc had a child during their time as a postdoctoral scholar at the University of Utah. The circumstances surrounding the pregnancy, birth, and early caregiving for the child took up a substantial amount of Dr. Postdoc’s time during that year, and an additional six months of postdoctoral training are requested in order to help Dr. Postdoc finish up their research program and move on to an independent position. Specifically, Dr. Postdoc will complete and submit the manuscript they are currently working on [include some details here].

Dr. Postdoc is currently applying for independent positions and anticipates moving on within the next year. An extension of the postdoctoral training period is requested to allow Dr. Postdoc to finish up their work here at the University of Utah and make a smooth transition to an independent position. Specifically, Dr. Postdoc will train a new lab member to carry out [some important procedure or technique] and publish [manuscript(s) in preparation].

Postdoctoral Fellow, Faculty Advisor, and Department or Unit Chair/Head Acknowledgement Section
The Postdoctoral Fellow, Faculty Advisor, and Department or Unit Chair/Head should sign and date the form as requested, acknowledging that:
- They are requesting approval to extend the Postdoctoral Fellow’s position beyond five years.
- They understand that this request can cover up to one additional year of postdoctoral training.
- The Faculty Advisor is committed to funding the Postdoctoral Fellow during the extension (or affirms that the Postdoctoral Fellow will be fully funded by a fellowship or other funding source).
- The Department or Unit Chair/Head’s signature confirms support for the extension request
but does not provide final approval for the request. Final approval rests with the
Graduate School Office for Graduate Education and Postdoctoral Affairs.
